What is an S1 Form?

If you’re planning to retire to Spain and receive a UK State Pension, or you’re already living in Spain and have reached UK State Pension age, you’ve probably come across something called an S1 Form.

For many British retirees, the S1 is one of the most important documents you’ll need, whether you’re applying for residency or you’re already legally resident in Spain. It allows you to access Spain’s public healthcare system.

Simply put, an S1 certificate allows the UK to remain responsible for funding your state healthcare while you receive treatment in another eligible country.

For retirees moving to Spain, this means you can access the Spanish public healthcare system under the same conditions as someone insured in Spain, while the UK reimburses Spain for the cost of your care.

Who is this guide for?

This guide focuses on UK State Pension recipients moving to Spain, or those already living in Spain who become entitled to their UK State Pension.

Although there are other situations where an S1 may be available, this guide focuses solely on retirees moving & living in Spain.

Why is the S1 so important?

One of the biggest concerns for many retirees is healthcare.

If you qualify for an S1, it will cover all your medical needs, just as it does back home, once your S1 has been registered in full.

You’ll become part of the Spanish public healthcare system, giving you access to healthcare in much the same way as Spanish residents.

Brexit and the S1

Brexit created a great deal of uncertainty for people planning to retire abroad.

Those already legally resident in the EU before the end of the Brexit transition period retained important healthcare rights under the Withdrawal Agreement.

For people moving after Brexit, the S1 system continued to exist, but one aspect caused considerable concern.

For several years, applicants were routinely advised that transferring their healthcare to Spain meant giving up their entitlement to NHS treatment when visiting the United Kingdom.

This understandably made many retirees nervous about requesting an S1, particularly those with ongoing medical conditions who regularly returned to the UK.

The 2024 S1 change many people still don’t know about

The position changed on 1 January 2024.

Eligible S1 holders are once again entitled to NHS treatment when visiting the UK under the current arrangements.

Many people are still unaware of this change because they remember the advice given in the years immediately following Brexit.

If you’re told something that doesn’t seem to reflect the current position, it’s always worth asking for confirmation against the latest NHS Business Services Authority guidance.

This means that transferring your healthcare responsibility to Spain no longer automatically implies giving up access to NHS treatment when visiting the UK.

Applying for an S1

Applying for an S1 has become much easier than it used to be.

People applying on the basis of their UK State Pension can now apply online through the NHS Business Services Authority, with digital copies often issued much more quickly than under the old paper-only process.

If online applications aren’t really your thing, don’t worry. The Overseas Healthcare Services team (+44(0)191 218 1999) are usually very helpful and can guide you through the application over the telephone, explaining each step as you go, although they will always advise you not to apply for it more than 3 months before moving.

Registering your S1 in Spain

Receiving your S1 is only the beginning.

Before you can benefit from Spain’s public healthcare system, your S1 must be registered with the relevant Spanish authority (the INSS).

If you are moving to Spain on a Non-Lucrative Visa, this step comes after your arrival — once your residency has been approved and completed, & you have received the documentation issued with it. Only when your S1 has been fully registered with the INSS can you complete your registration with the Spanish public healthcare system and be assigned your health centre and GP.

This final step matters more than many people realise. If you relied on your S1 to meet the healthcare requirement for your visa, your healthcare entitlement is not fully activated in Spain until this registration is complete — and you may encounter difficulties when renewing your residency if it has been left undone.

The same applies if you are already legally resident in Spain and become entitled to an S1 later — for example, on reaching UK State Pension age, or when moving from being a beneficiary of another person’s healthcare entitlement to holding an S1 in your own name. In each case, the S1 only takes effect in Spain once it has been registered with the INSS.
